(Registration Fee is non -refundable and due upon acceptance.)

School Entry Health Exam form – DH 3040 (From Physician)
Florida Certificate of Immunization – Form 680 (From Physician)
RE-REGISTRATION
Normal registration for existing preschool students takes place
each spring. There is a non-refundable registration fee of $140
for each child, which is payable at the time of registration.
Before any re-registration will be accepted, all payments due to
St. John Vianney Catholic School must be up-to-date or
arrangements must have been made with the Finance Committee for
a payment schedule.
Preschool students entering Kindergarten will need to fill out
the K-8th Grade Student Application Form and submit
it, along with a $30.00 application fee.
Acceptance into the SJV Preschool does not guarantee acceptance
into the SJV Kindergarten.
