St. John Vianney Catholic School

New Student Registration

Registration is held each March for those students wishing to attend St. John Vianney School the following fall.  Exact date/time is announced closer to registration time.  A test is given to all new students to determine placement.  All students entering St. John Vianney Catholic School for the first time are admitted on a probationary basis for a nine week period.  This is to ascertain a pupil's ability to adjust to the school philosophy and program.  The principal reserves the right to readmit students to St. John Vianney Catholic School. 

If you are applying for admittance, your child will be eligible for acceptance only after all information has been received and evaluated.  Before returning your registration packet, please make sure you have completed and included all the necessary paperwork. 

• Application Fee - $30.00 per student – grades K-8

(Application Fee is NON refundable)

• Check List included with Registration paperwork

• Family – Parent/Guardian Registration Form

• Student(s) Registration Form (one per student)

• Copy of Student’s Birth Certificate

• Copy of Student’s Baptismal Certificate

• Copy of most recent report card (grades K-8)

• Student Evaluation Form from prior school

• Pastor Approval Form (if applicable)

Upon acceptance the following additional information is needed:

•  Registration Fee - $125.00 per student – grades K-8

Registration Fee is non refundable and due upon acceptance

• Financial Commitment Form

• SMART Tuition Management Enrollment Form

• Florida Physical Form (From Physician)

• Proof of Immunization – 680 Form (From Physician)

Re-registration

Normal registration for existing students takes place each spring.  There is a non-refundable registration fee of $125 for each child, which is payable at the time of registration.  Before any re-registration will be accepted, all payments due to St. John Vianney Catholic School must be up-to-date or arrangements must have been made with the Finance Committee for payment schedule.

Important Note: Florida State Law requires that all children entering school for the first time must be immunized and must present the school with a Florida Certificate of Immunization or Exemption (HRS680) before school begins, or they cannot attend class.  This immunization form can be obtained from your doctor or county health department.
